Declaration of consent for processing of personal data
I hereby declare that:
1. I give consent for my personal data such as:
• name and surname / company name;
• address (street, house / flat number, zip code, city, country);
• e-mail address, contact phone number;
• NIP (Polish taxpayer number);
• bank account number;
and personal data regarding my child, such as:
• name and surname;
• class;
• allergies affecting the way of nutrition;
• products that are not consumed by a child;
by ART IMPRESSION CATERING SPÓŁKA Z OGRANICZONĄ ODPOWIEDZIALNOŚCIĄ entered into the
Register of Entrepreneurs conducted by Sąd Rejonowy dla m. st. Warszawy w Warszawie, XIII Wydział
Gospodarczy Krajowego Rejestru Sądowego, with legal seat in:ul.Powsińska 69/71, 02-903 Warszawa, KRS
0000217000, NIP 9512121898, REGON 015822495, share capital: 290.000 PLN („the controller”),
2. agree to process of personal data and my child's personal data
in order to execute the catering services contract concluded with the controller, including to execution of
possible claims related to the contract, to perform any legal, accounting and tax obligations of the controller
resulting from the contract,
3. I have provided my and my child personal data voluntary and correctly,
4. the processing of personal data takes place on the basis of my consent and for the purpose of rendering the contract,
5. I have been informed that I may withdraw my consent at any time,
6. I have been informed that my and my child personal data are processed is proceesed no longer than for the time
necessary to conclude and perform the contract and the period after which the claims under the contract will be
time-barred and for a period defined in legal regulations, e.g. the period in which regulations require us to store
invoices and accounting documents,
7. I have been informed that I have right to:
• request the controller to provide you with access to your personal data, and the right to correct, delete
or to limit the procesing of your personal data,
• raise an objection against prossesing of your data,
• ask for relocation of your data,
• file a complaint to the supervisory body - Prezes Urzędu Ochrony Danych Osobowych,
• withdraw of consent to the processing of your personal data; for this purpose, please contact the
controller via e-mail ado@ai-cateringwarszawa.pl,
8. I give consent to transfer of my personal data to third parties which co-operate with the controller to execute the
catering services contact, including entities providing the controller with legal, accounting, tax and IT services,
9. I have been informed that data will not be subjected to any form of automated processing of personal data
(profiling).
10. I have been informed that the consent is voluntary, but necessary for the conclusion and performance of the
contract.
